SELF-RETAINING SUTURE WITH AN ADJUSTABLE LOOP Russian patent published in 2021 - IPC A61B17/04

Abstract RU 2746457 C2

FIELD: medical equipment.

SUBSTANCE: group of inventions relates to medical equipment and namely to a self-retaining suture system for wound repair and two variants of a suturing method for wound repair. The system includes a needle having the first diameter and a suture having the first end for penetrating tissue. The first end is connected to the needle. The system also has an elongated suture body having a periphery and the first retainers on the periphery of the elongated body that are oriented towards the first end. The first retainers adhere to the suture body as the suture moves through tissue in the direction of the first end and resist the movement of the suture in tissue in a direction opposite to the direction of the first end’s location. The suture has the second end having an adjustable loop of an adjustable circumference. The adjustable loop has a fixed loop that slidably engages the elongated body to vary the circumference of the adjustable loop slidably. The fixed loop has no retainers on its surface. The fixed loop is formed by passing an end of the suture, which is not intended for placement, around itself and back and attaching this end to the body of the suture. This end should be located outside the fixed loop. The fixed loop has the second diameter, which is less than the first diameter and is configured to prevent the needle from passing through the fixed loop. The first end is adapted to pass through an adjustable loop to secure tissue as an anchor. The anchor prevents the suture from moving in the direction of the first end’s location. According to the first embodiment, the suturing method for wound repair includes providing a self-retaining system. The self-retaining system includes a surgical needle for suturing and a self-retaining suture. The self-retaining suture has a ligature having a ligature diameter, tissue retainers and an eyelet. The eyelet is formed by passing an end of the ligature, which is not intended for placement, around and back and attaching this end to the ligature in such a way so that it is located outside of the eyelet. The ligature passes through the eyelet to form an adjustable loop having the initial diameter. The needle is attached to the end of the self-retaining suture after the adjustable loop has been formed. The needle has the first diameter. The eyelet has the second diameter. The second diameter is smaller than the first diameter to prevent the needle from passing through the eyelet. The method includes inserting a needle into a patient's tissue at the first tissue location after the ligature has passed through the eyelet to form an adjustable loop. The method includes removing a needle from a patient's tissue at the second tissue location. The method includes passing the needle and at least part of the ligature through the adjustable loop. The method includes inserting a needle into a patient's tissue at the third tissue location. According to the second embodiment, the method includes passing the ligature through the eyelet to form an adjustable loop having an initial diameter after providing a self-retaining system.

EFFECT: invention improves the ability of fixation in the surrounding tissue, improves the characteristics of tissue retention, increases the maximum load and improves clinical performance.
