FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to ophthalmology, and can be used to predict the course of reparative processes in the outflow tracts of intraocular fluid in patients with open-angle glaucoma after non-penetrating deep sclerectomy. The dynamics of the concentration of TGF-β and MMP-9 in the lacrimal fluid of patients in the postoperative period is assessed. The dynamics of the concentration of TGF-β and MMP-9 in the lacrimal fluid of patients is determined before surgery. The dynamics of the concentration of TGF-β and MMP-9 in the lacrimal fluid of patients is also determined 14 days after non-penetrating deep sclerectomy. In the case of a moderate increase in TGF-β of a tear, equal to 280-350 pg / ml, and the MMP-9 content in a tear, equal to 83-174 ng/ml, a favorable course of reparative processes in the outflow tracts of the intraocular fluid is predicted, which does not require correction of the anti-inflammatory therapy regimen. When TGF-β is less than or equal to 189 pg / ml, and MMP-9 in tears is equal to 4.25-82.5 ng/ml, a moderately favorable course of reparative processes is predicted, requiring either strengthening and prolongation of local instillation anti-inflammatory therapy, or performing the needling of the filtering bleb. With a significant increase in the concentration of TGF-β of a tear, equal to 600-750 pg / ml, and the level of MMP-9 of a tear up to 2.6 ng/ml, the course of reparative processes is considered conditionally unfavorable in terms of the hypotensive efficacy of antiglaucoma surgery and there is a need for active anti-inflammatory and anti-fibrotic therapy.
EFFECT: method enables the development of an algorithm for predicting the course of reparative processes in the outflow tracts of intraocular fluid in patients with open-angle glaucoma after non-penetrating deep sclerectomy and further planning of therapeutic measures aimed at limiting the fibrous transformation of the newly created outflow tracts, by determining the dynamics of the concentration of TGF-β and MMP-9 in the lacrimal fluid of patients before surgery, as well as 14 days after non-penetrating deep sclerectomy.
