METHOD FOR USING GUARANTEED TIDAL VOLUME IN NEWBORNS WITH SEVERE RESPIRATORY PATHOLOGY, MANIFESTED BY RESPIRATORY FAILURE OF II-III SEVERITY Russian patent published in 2021 - IPC A61M16/00

Abstract RU 2750841 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to neonatology and resuscitation, and can be used in the treatment of severe respiratory pathology, manifested by respiratory failure of II-III severity. For this, adaptive "intelligent artificial ventilation of the lungs (AVL) is used with the maintenance of a given ventilation volume. Initially, the parameters of the guaranteed tidal volume are set, taking into account the pathology, the level of the oxygen fraction FiO2 is set depending on the value of the percutaneous saturation SpO2 in the range 0.21-1.0, using the CliO2 feedback technique with the level of the oxygen fraction FiO2, the respiratory rate - in the range 30- 50 breaths per minute, taking into account the starting ratio of inspiratory time and expiratory time 1: 2, the value of the inspiratory time is set according to the body weight, depending on the gestational age, the value of the pressure level during inspiration Pinsp is set depending on the pulmonary pathology, the level of pressure in the expiratory airway (peep) is set in the range of 4-6 cm of water column., the peak pressure in the respiratory tract set in the range of 25-60 cm of water column. Continuously, 24 hours a day, vital signs are monitored according to the calculated parameters issued by the ventilator, such as: compliance (Cdyn/kg), mean airway pressure (pmean), airway resistance level (Raw), control of graphical monitoring of flow curves, graphical loops of tidal volume versus airway flow, and airway pressure versus tidal volume. A daily ultrasound examination of the lungs is carried out, the blood gas composition and the acid-base balance of the blood are monitored 1-2 times a day, with a chest x-ray.

EFFECT: invention provides a decrease in ventilation parameters in the first six hours after connecting a newborn to the ventilator, reducing the time the patient spends on ventilator up to 3 days, which allows to significantly neutralize the negative effect of artificial lung ventilation on the respiratory system and other organs and systems of the newborn, and significantly increase the survival rate of the newborn.
