FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to oncology and can be used in the treatment of squamous cell p-16 positive oropharyngeal cancer. The method includes induction chemotherapy followed by chemoradiotherapy. Three weeks before chemoradiotherapy, induction therapy is performed according to the following scheme: on day 1 intravenously: paclitaxel 80 mg/m2 for 60 minutes + carboplatin AUC2 for 60 minutes + cetuximab at a dose of 400 mg/m2 for 120 minutes, on days 8, 15, 22, 29 and 36 - intravenously: paclitaxel 80 mg/m2 for 60 minutes + carboplatin AUC2 for 60 minutes + cetuximab 250 mg/m2 for 60 minutes. When the tumor is resorbed after 3 weeks, the 2nd stage of treatment is carried out: the introduction of cetuximab 250 mg/m2 1 time a week in combination with radiation therapy to the locoregional area, including regional lymphocollectors in the fraction dose 1.8 Gy daily up to the total radiation dose of 54 Gy, and to the area of the primary tumor and metastatically altered lymph nodes 1.8 Gy per day for three and a half weeks and then 1.8 Gy and 1.5 Gy boost after 4-6 hours for two and a half weeks up to the total radiation dose 72 Gy.
EFFECT: use of the invention makes it possible to increase the rate and degree of regression of the primary tumor, to suppress subclinical disseminated metastases due to the combined chemoradiation effect on the tumor.
