SUBSTANCE: invention relates to medicine, namely to ophthalmology. For the surgical treatment of angle-closure glaucoma with the presence of pupillary and organic angular blocks, a conjunctival incision, coagulation of episcleral vessels, formation of superficial and deep scleral flaps, removal of a deep scleral flap, formation of a drainage hole and completion of the operation are performed. Before the formation of the drainage hole, the surface of the ciliary body is flattened at the junction with the root of the iris using an optical probe with an outer diameter of 2 mm and a working spot size of 1500 mcm and a diode laser with a wavelength of 810 nm and an output power of 0.3 W. Next, using an optical probe with an outer diameter of 900 mcm, and a working spot size of 500 mcm and a power of 0.2 W of the above laser, a drainage hole is formed at the junction of the iris root and the surface of the ciliary body until filtration from the anterior and posterior chambers appears.

EFFECT: invention reduces the total radiation power, ensures the formation of a hole with clearer contours due to the directional effect of laser radiation, and also reduces inflammation in the postoperative period by creating one drainage hole with clear edges, which makes it possible to achieve a stable hypotensive effect in the long-term observation period.
