SUBSTANCE: conjunctiva is separated, episcleral vessels are coagulated, a superficial scleral flap is formed, a deep scleral flap is excised, and a drainage hole is formed using a diode laser with a wavelength of 810 nm using an optical probe with an outer diameter of 0.9 mm and a working spot size of 0.5 mm. Before the formation of the drainage hole, the entire exposed surface of the ciliary body (CB) is evenly flattened with the same laser using an optical probe with an outer diameter of 2 mm and a working spot size of 1.5 mm. The flattening of the exposed surface of the CG is carried out with an output power of 0.4–0.6 W. Then, a 2-mm incision is made with a blade in the circular ligament at the transition point of the iris to the CT, the iris is removed from the incision with the help of iris-tweezers, and the fragment of the iris inserted into the incision with a part of the CT is excised with Vannas scissors. A drainage hole is formed around the incision with the same laser at a laser output power of 0.2–0.4 W, a pulse duration of 4–6 s. Then, using a laser with the same parameters, the drainage hole is expanded to the projection zone of the ciliary sulcus until filtration from the posterior chamber of the eye appears.

EFFECT: obtaining stable compensation of ophthalmotonus immediately after surgery and stabilization of IOP in the long-term follow-up period.
