METHOD FOR SURGICAL MANAGEMENT OF CLOSED-ANGLE GLAUCOMA WITH ANTERIOR CHAMBER ANGLE BLOCK Russian patent published in 2013 - IPC A61F9/00

SUBSTANCE: invention refers to medicine, namely to ophthalmology, and may be used for surgical management of closed-angle glaucoma with anterior chamber angle (ACA) block. A 2 mm conjunctival incision is made. That is followed by episcleral vessel coagulation. A superficial scleral flap 2.5x2.5 mm is cut out at one-third of the scleral thickness. A drainage hole of the diameter of 1 mm is formed with a laser at wave length 810 nm and output power 0.5 to 0.6 Wt, pulse length 3-5 s. After the superficial and deep scleral flaps have been formed, the organic ACA block is accompanied by removing the deep scleral flap to a round ligament. A drainage hole is formed in the round ligament in a projection of a junction of the iris and ciliary body. The laser of the same characteristics is used to drift the drainage hole to visualise the iris root, while the laser of power 0.4 Wt and pulse length 3 seconds forms a root coloboma until a filtration is observed from the anterior chamber. The functional ACA block required forming the drainage hole in a projection of the ciliary body until the filtration is observed from the posterior chamber, after the superficial and deep scleral flaps have been formed. The deep scleral flap is removed to exposure a trabeculo-Descemet's membrane and to observe the filtration therefrom.

EFFECT: method provides reducing a risk of postoperative complications, hastening the recovery, reaching the prolonged hypotensive effect in the remote follow-up period.
