An oblique-transverse diaphragmotomy is performed, the suprahepatic section of the inferior vena cava is taken intrapericardially, the Cattel-Braasch maneuver is performed - right-sided medial visceral rotation and at the point where it flows into the inferior vena cava, the arteries and veins are ligated and transected, the renal and vein bases are transected, the inferior vena cava is clamped, ligated and transected 3 cm above its bifurcation, then the right lobe of the liver is mediated, the short hepatic veins are cut off, the thrombus is squeezed from the inferior vena cava to a level below the caval gate and above the tumor thrombus, a Satinsky clamp is applied, then the kidney with the formation and section of the inferior vena cava with the tumor thrombus is removed as a single block without subsequent reconstruction. In the presence of metastatic tumor nodes, para-aortic lymphadenectomy is performed.

EFFECT: allows extirpation of the retrohepatic part of the inferior vena cava together with a tumor thrombus without its subsequent reconstruction, which significantly reduces the risks of postoperative complications and makes it accessible and applicable in the age category of patients, as well as in patients with concomitant pathology.
