SUBSTANCE: invention relates to medicine, namely to operative urology. The operation is performed using a holmium laser fiber by dissecting the mucous membrane of the prostatic urethra and prostate tissue from the apical zone to the bladder neck in the proximal direction along the anterior semicircle to the surgical capsule in the direction of 13 and 23 o'clock of the conventional dial. A triangular area is preserved, including the mucous membrane and underlying tissues of the prostatic urethra along the anterior semicircle of the posterior urethra with the base of the conditional triangle on the bladder neck and the apex at the transverse fibers of the external sphincter.

EFFECT: method allows maximally preserving the integrity of the urethral sphincter in the anterior region, thereby eliminating the surgical risk of developing stress urinary incontinence in the postoperative period, as well as creating optimal surgical access to hyperplastic prostate nodes and their subsequent enucleation.
