FIELD: medicine; operative urology.
SUBSTANCE: cystostomy drainage balloon is preliminarily lowered behind an apical zone of the prostate and filled with 0.9% sodium chloride solution up to 3 ml. With constant pulling of the drainage through the cystostomy passage, club-shaped incisions of the prostatic urethra are made transurethrally by dissecting the mucous membrane and tissue from the apical zone to the bladder neck in the proximal direction along the anterior semicircle in direction of 5 and 7 o'clock of the conventional clock face, keeping a portion of the urethral mucosa at 12 o'clock. Method includes traction of hyperplastic tissues of prostate and their further enucleation with holmium laser with power of laser radiation of 120 W, current intensity of 1.4 J and pulse frequency of 60 Hz in incision mode and current intensity of 2.0 J, frequency of 40 Hz in coagulation mode or using plasma kinetic energy for enucleation with ignition power of 300 W and coagulation mode of 150 W.
EFFECT: method allows maximally preserving the integrity of the urethral sphincter, preserving the anatomical structure of the layer during enucleation with adequate tissue visualization and thereby reduce the surgical risk of developing stress urinary incontinence in the postoperative period, as well as to improve surgical access to hyperplastic prostate nodes and their subsequent enucleation using cystostomy drainage.
