FIELD: medicine.
SUBSTANCE: invention relates to medicine, to vertebrology, to traumatology and orthopedics, neurosurgery, namely spinal surgery, and can be used to insert screws into the first cervical vertebra in case of hypoplasia of the C1 vertebral arch and abnormal course of the vertebral artery. The posterior structures of the C1-C2 vertebrae are skeletonized from the dorsal approach. Microsurgical mobilization of the vertebral artery is performed. In the region of the groove of the vertebral artery on the posterior arch of the C1 vertebra, the vertebral artery is cranially removed with a neurosurgical raspatory. Above the posterior arch of the C1 vertebra, an instrument is installed into the groove of the vertebral artery and a canal is formed, orienting it along the groove of the vertebral artery into the lateral mass of the vertebra. A screw is inserted into the canal, the diameter of which is 3.5 mm.
EFFECT: method provides safe introduction of screws into the first cervical vertebra in case of hypoplasia of the C1 arch and abnormal course of the vertebral artery and the possibility of fixing the C1 vertebra with a transpedicular fixation system due to the introduction of screws into C1 partially or completely over the posterior arch of the vertebra.
