SUBSTANCE: invention can be used to restore the balance of the spine in congenital and systemic deformities of the cervical spine. Surgical access to the cervical spine is performed, deformity is corrected and a transpedicular fixation system is installed. With the patient in the lateral position opposite the hemivertebra, a vertical incision is made in the projection of the hemivertebra, the transverse process of the hemivertebra is isolated, and the underlying spinal nerves are visualized. The transverse process of the hemivertebra is removed and the vertebral artery is isolated. After mobilization of the nerves and vertebral artery, the hemivertebral body is removed, adjacent discs are curetted, and the posterior longitudinal ligament is dissected. The wound is sutured. The patient is turned onto his stomach. A midline linear incision is made in the projection of the segments above and below the hemivertebra. The cervical vertebrae above and below the hemivertebra are skeletonized. Bone screws and longitudinal rods of the transpedicular fixation system are installed, the semi-arch of the hemivertebra is removed, and the deformity is corrected. The rods of the transpedicular fixation system are fixed with nuts. Spinal fusion is created using autobone.
EFFECT: method provides the possibility of reconstructing deformation of the cervical spine with a lower risk of neurological complications and a small amount of blood loss due to the lateral approach, which allows more precise skeletonization and visualization of neurovascular structures, while increasing the working field.
