SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics; it can be used in the treatment of major osteochondral articular defects, intra-articular fractures with a preserved osteochondral fragment. After installing the osteochondral fragment in the area of the intended installation of a fixator, a cylindrical full-layer section is taken from the osteochondral fragment, the diameter of which corresponds to the diameter of the intended fixator, and the height is no more than 50% of the depth of the defect. Through a hole formed after the take of the full-layer section, the osteochondral fragment is fixed to the joint. A cylindrical full-layer graft is taken from the unloaded area of the joint, the height of which corresponds to the height of the full-layer section previously taken from the osteochondral fragment, and the diameter is 1-1.5 mm larger than the diameter of the previously taken section. Next, the graft is placed in the fixator installation area.

EFFECT: method allows one to preserve the integrity of the cartilage tissue and provides the most favorable biomechanics of the joint due to the additional fixation of the fragment with an immersion graft in place of the section taken at the beginning of the operation, while the installed fixator is isolated from the cartilage surface of the joint, preventing the chondrotoxic effect of material, of which it is made.
