FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely orthopedics, and can be used in preoperative planning and carrying out the reduction of the greater trochanter during hip joint endoprosthetics. On the radiograph of the hip joints in a direct projection, the magnitude of the displacement of the greater trochanter of the altered femur in the frontal plane (A) is determined. Then, using an X-ray with a hip joint replacement template attached to it, the distance d is determined, which is a segment drawn from the tip of the greater trochanter of the modified femur to the line passing through the center of rotation of the head of the pre-selected femoral component template. The angles of the anterversion of the modified and unchanged femurs are determined and their difference, angle C, is calculated. An isosceles triangle is built with sides d and the angle of the vertex C. The value of the base of the isosceles triangle is calculated, which is equal to the value of the displacement of the large spit in the horizontal plane (B). Access to the hip joint is provided. Then, from the level of the transverse osteotomy of the femur and perpendicular to it, a line of length A is drawn. Using this line as a guide, after installing the acetabulum and femoral components of the endoprosthesis, the greater trochanter, together with the gluteal muscles, is lowered in the frontal plane by value A and in the horizontal plane by value B.
EFFECT: method makes it possible to improve the quality of life of patients with coxarthrosis, reduce the rehabilitation time and eliminate repeated operations due to the possibility of accurate positioning of the greater trochanter.
