FIELD: traumatology and orthopedics.
SUBSTANCE: inventions group relates to traumatology and orthopedics and can be used to stabilize the shoulder joint during its chronic instability. The implant for replacing the bone defect zone on the anterior surface of the glenoid of the shoulder blade has the shape of an anatomically curved trapezoidal prism with a smoothed outer contour and is made individually from an ultrapure titanium-aluminum-vanadium alloy (Ti6Al4VELI) by selective laser fusion on an SLM 3D printer using a three-dimensional model. The three-dimensional model was created when designing the anatomical shape of the implant using hybrid parametric modeling and topological optimization taking into account the parameters of the glenoid bone defect according to multispiral computed tomography of the shoulder joint. The implant has a lower contact wall with conical spikes, curved inward and an upper wall, going with an inclination, curved outward at an angle. The upper and lower walls have technological holes with a specific location, diameter, depth and direction obtained during the design for the installation of fixing screws and Kirschner wires into the channels with the direction and depth of the glenoid conduction in the bone of the implant opening. In the center there is one blind hole with a support platform and a hexagonal recess for the counter part of the manipulator, opposite which a pin is located on the lower contact wall, the height of the front wall is greater than the rear one, the side walls are directed towards at an angle α obtained during the design. The manipulator for installing the implant includes a rod in the form of a cylinder with a section truncated by a plane parallel to its longitudinal axis to form a rectangular side wall. At the working end, the rod has a hexagonal protrusion, with a split retaining ring installed in a circular groove to interact with the counterpart of the implant. The opposite square end engages the sleeve. The rod is inserted into a tube on which an L-shaped stop with a cylinder-shaped rod retainer is installed. EFFECT: implant and manipulator allow to reliably and stably fix the implant on the manipulator when positioning the implant in space in all directions and not to limit visual control in the narrow space of the operating field, including muscles and tendon complex, to reduce the time for the operation and to simplify it in general.
