METHOD FOR ARTHROSCOPIC AUTOPLASTY OF SIGNIFICANT BONE DEFECT OF GLENOID IN CHRONIC ANTERIOR SHOULDER INSTABILITY USING 3D PRINTING TECHNOLOGY Russian patent published in 2024 - IPC A61B17/56

SUBSTANCE: invention relates to medicine, namely to traumatology and orthopaedics, and can be used for arthroscopic autoplasty in bone defect of glenoid more than 40% in chronic anterior shoulder instability. An autograft is extracted from an iliac wing and decorticated. A coracoid process is separated with preserving a tendon of a short head of a biceps muscle and a coracobrachial muscle. Then, the iliac wing autograft is adapted with the base represented by the spongy bone to the prepared bed in the defect zone on the anterior glenoid and temporary fixation with two pins drilled from front to back through autograft and glenoid. An osteotomy of the coracoid process is carried out, then the osteotomised coracoid process with a tendon complex is adapted to the prepared autograft bed and permanent fixation is performed with two cannulated screws delivered from front to back through both autografts into an articular process of a scapula. In the postoperative period, computed tomography is performed, the results of which are used to design and print 3D model of the scapula, which is used for postoperative control.

EFFECT: method provides the clinical effectiveness in the patients with a significant volume of the glenoid bone tissue defect in chronic anterior shoulder instability due to arthroscopic defect filling with free bone autograft from iliac wing and osteotomised coracoid process with tendon complex simultaneously, strong fixation of the structure, preservation of congruence of surfaces of grafts with articular surface of glenoid and head of humerus.
