FIELD: medicine.
SUBSTANCE: invention relates to medicine, in particular to neurosurgery and traumatology. The perforator of the posterior longitudinal ligament of the spine for use in ventral subaxial cervical spondylodesis contains connecting screws, a leaf spring, a cutting blade, a receiving blade. The perforator is made in the form of a pistol and consists of an integral monolithic handle and a hollow body located relative to the monolithic handle at an angle of 135 degrees, and collapsible parts: a hollow body cover, a replaceable wire mandrel, a trigger with a spike, a handle, a dog, a collet, a nut. As a receiving blade, the perforator is equipped with a supporting branch with an anvil with a pointed tip, and as a cutting blade, with a guillotine placed on the supporting branch. A channel is made in the support branch, ending with an outlet at the tip of the anvil for pushing the replaceable wire mandrel out of it. The guillotine is made with a proximal toothed part. The distal angle at the base, relative to the handle, of each of the teeth of the guillotine is sharper than the proximal one. The contact surfaces of the guillotine and the support branch are equipped with thresholds, one each. The thresholds are located in opposite directions. The surface of the guillotine, opposite to the empty one, is equipped with a flag. The proximal toothed part of the guillotine is immersed in a hollow body and closed with its lid. Next to the monolithic handle is a handle, on the axis of rotation in the form of a connecting screw. A dog is pivotally connected to the handle to let the dog bend in the direction of the guillotine blade and with the possibility of withdrawing and raising the guillotine blade using a flag, from the anvil to the length of the toothed part of the guillotine with the handle pressed against the handle. The dog and the part of the handle connected to it by means of a connecting screw are hidden under the cover of the hollow case. The leaf spring is located in the hollow housing, at the distal side of the dog, and ensures the return of the handle and the dog to its original position when compression stops. The collet sleeve is made with a hollow body to accommodate the said mandrel with the possibility of clamping the mandrel with a nut and with a groove in the neck for the trigger spike. The nuts and collets of the sleeve are located outside the hollow housing. A spiral spring is located between the nut and the housing on the collet sleeve, which ensures the return of the trigger and collet sleeve with a replaceable wire mandrel clamped into it to its original position. The channels of the collet sleeve and the support branch are located on the same geometric axis, while a trigger with a spike located on the connecting screw, which is the axis of rotation for it, is located distal to the above handle.
EFFECT: invention provides an improved view of the intervention area, the perforator acquires a chopping and cutting function, which makes it possible to form a defect with a smooth edge, and also there is the possibility of forming a defect of the posterior longitudinal ligament without changing the tool.
