SUBSTANCE: invention relates to medicine, namely to thoracic surgery. A lateral thoracotomy is performed in the 4th intercostal space. The pulmonary artery, superior and inferior pulmonary veins are ligated and divided. The main bronchus is isolated. A lymph node dissection is performed. Before applying a mechanical suture, a fragment of a polypropylene mesh measuring 2.0×5.0 cm is placed under the upper jaw of the stapler. Then the main bronchus is sutured with a hardware suture at the level of the tracheal bifurcation with simultaneous fixation of a fragment of a polypropylene mesh with a mechanical suture to the bronchus stump. The main bronchus is crossed distally to the mechanical suture. The lung is removed. Excess polypropylene mesh is cut off. The flap of the mediastinal pleura is mobilized from the lateral surface of the pericardium while maintaining blood supply. The mobilized flap of the mediastinal pleura is fixed with a continuous suture to the edges of the mediastinal defect, covering the bronchus stump.

EFFECT: method makes it possible to achieve the formation of scar tissue in the region of the bronchus stump, reduce the incidence of failure of the stump of the main bronchus and reduce the likelihood of formation of a bronchopleural fistula after pneumonectomy.
