SUBSTANCE: invention relates to medicine, namely to otorhinolaryngology. Surgical access to the cholesteatoma is carried out with subsequent exposure to laser radiation by evaporation. In this case, the cholesteatoma is separated from the surrounding tissues. Then it is evaporated using a TruBlue WOLF laser with a wavelength of 445 nm, a power of 1-2 W, non-contact in a pulsed mode, the pulse duration is 20-40 ms, the time between pulses is up to 100 ms. The distance from the end of the light guide to the surface of the cholesteatoma is gradually reduced, starting from 4 mm to 0.5 mm, until the cholesteatoma is reduced by 80%. After that, the cholesteatoma is completely removed by contact in the mode of continuous exposure to laser radiation with a wavelength of 445 nm with a power of up to 2 W.

EFFECT: method enables radical removal of the pathological process, prevention of damage to the surrounding neurovascular structures, minimization of the risks of developing cholesteatoma of the temporal bone pyramid in the long-term period, improvement of functional results in relation to hearing and functioning of the facial nerve, and reduction of the patient's stay in the hospital.
