SUBSTANCE: invention relates to medicine, in particular to pediatric surgery. In the surgical treatment of a tracheoesophageal fistula with a diameter of more than 0.5 cm after a burn of the esophagus with a disk battery in children in serious condition with respiratory failure, the treatment is carried out in two stages. At the first stage, after mobilization of the esophagus and trachea, the esophagus is crossed above and below the tracheal defect, keeping the esophagus in the area of ​​the fistula. The distal esophagus is resected and its stump is sutured over the diaphragm. Then necrectomy in the area of ​​the tracheal defect is performed. An esophageal flap is formed from the esophageal section preserved in the fistula area according to the size of the tracheal defect with preservation of its own local blood supply and sewn with interrupted sutures along the edges of the tracheal defect, closing it. Along the course of the sternocleidomastoid muscle, the skin, underlying tissues are dissected on the left, the proximal esophagus is isolated, its resection is performed, and an esophagostomy is formed on the neck using the created access. A gastrostomy is formed. At the second stage, a coloesophagoplasty is performed.

EFFECT: method makes it possible to reliably form the posterior wall of the trachea, prevent tracheal suture failure and cicatricial narrowing of the trachea, minimize the risk of fistula recurrence and mediastinitis, simplify surgical intervention as much as possible, and improve the patient's quality of life and social rehabilitation.
