FIELD: medicine.
SUBSTANCE: invention relates to the field of medicine, namely to X-ray endovascular surgery. Fenestration of the partially opened stent graft is performed by a fenestration device at a point located proximally or in the same horizontal plane with the mouth of the target artery, followed by the introduction of a conductor through the created lumen in the stent graft into the proximal segment of the aorta. After the installation of the conductor, its externalization is performed through radial or shoulder access. A balloon catheter is installed along the externalized conductor and dilation of the stent graft tissue is performed to increase the diameter of the fenestration. Then the balloon catheter is removed, then a conductor catheter is installed along the conductor through the created fenestration in the stent graft and selective catheterization of the lateral branch is performed. Next, a second conductor is installed through the conductor catheter into the distal segment of the target artery. A peripheral stent graft is passed along the conductor inserted into the target artery, and its positioning is performed. The next stage is the final implantation of the aortic stent graft and then the peripheral stent graft is implanted into the lateral branch with an exit of 3-5 mm into the lumen of the aortic stent graft.
EFFECT: method allows one step during the endoprosthetics of the aorta or iliac arteries to perform fenestration of the stent graft with subsequent implantation of the stent graft from the position of the main branch - the lateral branch of the aorta or iliac arteries, as well as to reduce the risks of complications associated with perforation of the arteries during fenestration.
