FIELD: medicine.

SUBSTANCE: autovenous bypass is applied between the superficial temporal artery and the medial cerebral artery. the superficial temporal artery trunk is separated opposite the tragus; a bone flap is cut out; the dura mater is dissected, and the M4-segment branch of the medial cerebral artery of the maximum diameter is separated. That is combined with sampling a fragment of the great saphenous vein from a lower leg, and the autovenous bypass is formed. The superficial temporal artery trunk is temporarily clipped; an outer wall is dissected; the autovenous bypass is sutured end-to-side therein with a blanket suture. The autovenous bypass trunk is temporarily clipped, whereas the superficial temporal artery is unclipped; the separated M4-segment branch the medial cerebral artery is mobilised throughout 8-12 mm and temporarily clipped; the wall is dissected lengthwise, and the autovenous bypass is sutured end-to-side therein with the blanket suture. That is followed by unclipping, determining the pulsation of the autovenous bypass, and the blood filling of the afferent and efferent M4 segments of the medial cerebral artery.

EFFECT: method enables reducing postoperative complications and increasing clinical effectiveness that is ensured by reducing the damaging involvement of the brain cortex.
