FIELD: medicine; traumatology; orthopedics; functional diagnostics.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, functional diagnostics; it can be used in therapeutic blockade for plantar fasciitis. For this purpose, extra-perineural conduction anesthesia of the main trunk of the gastrocnemius nerve is preliminary performed. A place of administration of anesthetic is determined by ultrasound verification of gastrocnemius nerve, placing an ultrasound sensor in the transverse plane on the posterior surface of the shin at the boundary c/3-n/3 of the shin, and moving the sensor in the range of 10-15 cm in a distal and proximal direction until ultrasound signs of the peripheral nerve appear, characteristic for the main trunk of the gastrocnemius nerve. Such signs include an ovoid structure consisting of small hypoechoic inclusions with hyperechoic areas of a “honeycomb” type in the transverse plane, by which it is concluded about the detection of the gastrocnemius nerve near the small saphenous vein (V.Saphenaparva) with a variable position relatively to it. Then, at a place, where small hypoechoic inclusions with hyperechoic areas of a “honeycomb” type are the most clearly verified, the injection of a traumatic needle is performed under ultrasound control with its subsequent movement in a lateral-medial direction at an angle of 5-10 degrees relatively to the longitudinal axis of the sensor to the extra-perineural area of the verified nerve. After that, conduction anesthesia is performed using local anesthetic.
EFFECT: method provides effective treatment of plantar fasciitis with minimization of painful sensations due to the most accurate supply of anesthetic to plantar fascia, directly to the nerve.
