METHOD OF TREATMENT OF RESISTANT PAIN IN THE MEDIAL AND PLANTAR AREAS OF THE HEEL REGION Russian patent published in 2023 - IPC A61B18/04

Abstract RU 2801602 C1

FIELD: medicine; traumatology; orthopedics.

SUBSTANCE: invention can be used to treat resistant pain in the medial and plantar areas of the heel region. Before the RFD procedure of the calcaneal region, a landmark for the passage of Baxter's nerve is found — the intersection point of two lines: a line drawn parallel to the posterior edge of the inner ankle, and a line drawn parallel to the plane of the sole of the foot at the level of the Achilles tendon attachment. Under ultrasound guidance, a 22G radiofrequency cannula 10 cm long with a 5 mm active tip is inserted at the point of intersection of the lines. The tip reaches the space between the deep fascia of the abductor muscle and the medial head of the square plantar muscle. A second similar cannula is inserted 5 mm distal to the first one, sensory and motor sensitivity is checked at these points using Cosman G4 RFD apparatus in bipolar mode, then Novocaine solution 0.5% — 2.0 is injected into each cannula and radiofrequency thermal ablation is performed at a temperature of 85°C for 90 seconds.

EFFECT: method ensures the achievement of a stable clinical effect immediately after the procedure without the risk of any complications and functional limitations due to the precise impact of the procedure on the first branch of n. plantaris lateralis due to ultrasound navigation and a bipolar system of radiofrequency denervation and an increase in the area of thermocoagulation.
