FIELD: medicine; clinical immunology; hematology.

SUBSTANCE: invention relates to medicine, namely to clinical immunology and hematology; it can be used for the prevention of rejection of a hematopoietic stem cell transplant and associated complications in children with primary immunodeficiency. The method includes intravenous injection to a patient of a preparation of a granulocyte colony-stimulating factor, at a single dose of 10 mg/kg, from the first to the fifth day of preparatory therapy before transplantation. From the third to the fifth day of therapy, the patient is subcutaneously injected with plerixaphore, at a single dose of 240 mkg/kg. At the same time, the patient is conditioned using chemotherapeutic preparations of a group of fludarabine antimetabolites, at a course dose of 150 mg/m2, and melphalan alkylating agents, at a course dose of 150 mg/m2, and treosulfan, at a course dose of 42 g/m2, a preparation of chimeric monoclonal anti-CD20 rituximab antibodies, at a course dose of 100 mg/m2, and antithymocyte immunoglobulin, at a course dose of 5 mg/kg. On the 9th day, hematopoietic stem cells are transplanted with a TCRαβ/CD19-depleted transplant.

EFFECT: use of the invention allows for prevention of the development of a reaction of rejection of hematopoietic stem cells in children with primary immunodeficiency due to fast, reliable engraftment of a hematopoietic stem cell transplant, reduction in the risk of toxic complications, and improvement of the quality of life of patients after transplantation.
