FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to plastic surgery. The duration of the operation, body mass index, area of surgical action, risks of anesthesia according to the ASA scale, factors predisposing to thromboembolism are assessed. In this case, the volume of liposuction is additionally determined, expressed as a percentage of body weight. The core temperature of the patient's body is monitored throughout the operation. The patient's blood hemoglobin level before and during the operation and the smoking status are determined. In this case, the duration of the operation up to 4 hours is estimated at 1 point. The duration of the operation is 4-6 hours - 2 points, more than 6 hours - 4 points. The need for one or two small additional operations, or one medium, or one large additional operation is estimated at 1 point, two additional medium or large operations - 2 points, 3 or more additional operations - 4 points. Wherein minor additional operations include: otoplasty, gynecomastia correction, liposuction up to 300 ml, blepharoplasty. For medium surgery - reduction or enlargement of the mammary glands, rhinoplasty, liposuction up to 1000 ml, dermolipectomy of the upper limb, mini-abdominoplasty, mini-facelift, for a large operation - abdominoplasty, liposuction up to 5000 ml, torsoplasty, dermolipectomy of the lower limb. The value of body mass index 18.0-29.9 is estimated at 1 point, 30-35.0 - 2 points, more than 35 - 4 points. The area of operative action up to 20% of the body surface - 1 point, from 20% to 30% - 2 points, from 30% to 40% - 4 points. Anesthesia risks: with ASA I - 1 point, with ASA II - 2 points, with ASA III - 4 points. Factors predisposing to thromboembolism: in the presence of one factor - 1 point, with 2 factors - 2 points, with 3 or more - 4 points; the volume of liposuction, expressed as a percentage of the aspirated proportion of adipose tissue of body weight: with a value of less than 5% - 1 point, with 5-7% - 2 points, with a value of more than 7% - 4 points; monitoring the patient's core temperature during the operation: at a temperature of 36.6-36.0°C - 1 point; 35.9-35.0°C - 2 points; less than 35.0°C - 4 points; the patient's blood hemoglobin level: before surgery more than 120 g/l, during surgery more than 90 g/l - 1 point, before surgery 100-120 g/l, during surgery 70-90 g/l - 2 points, before surgery less than 100 g/l, while less than 70 g/l - 4 points. No smoking habit - 1 point, abstinence from smoking for 4 weeks or more - 2 points, the patient continues to smoke - 4 points. The scores are summed up and with a score of 10-16, the risk of surgery is assessed as low. With a score of 17-20 - as an average. More than 20 - how high.
EFFECT: method allows to improve the patient's safety and the quality of complex operations for aesthetic correction of contours.
