FIELD: medicine, ophthalmology.
SUBSTANCE: morphological and functional state of the retina of the macular region is analyzed in the pre- and postoperative period of combined kidney and pancreas transplantation of the following indicators: central retinal thickness, vascular density and perfusion in the superficial and deep capillary plexus of the retina using an optical coherence tomography scanner with angiography mode by NIDEKRS-3000 Advance 2 and an average threshold of light sensitivity using MacularIntegrityAssessment fundus micropyrometer. At the same time, the retina of the macular area is scanned on an optical coherence tomograph NIDEKRS-3000 Advance 2 in the MaculaMap 3.3x3.3 mm AngioSan mode, the threshold of light sensitivity is determined with the help of the MacularIntegrityAssessment fundus micropyrometer in patients in the pre- and postoperative period in terms of drug-induced mydriasis. Moreover, a decrease in the central thickness of the retina by 20% or less, an increase in the density of blood vessels in the superficial capillary plexus of the retina by 21% or more, in the deep retinal capillary plexus by 26% or more; an increase in perfusion density in the superficial capillary plexus of the retina by 20% or more, in the deep retinal capillary plexus by 24% or more; the increase in the average threshold of light sensitivity by 16% or more in the postoperative period of combined kidney and pancreas transplantation allow us to conclude that diabetic retinopathy has been stabilized with a tendency to improve the morpho-functional state of the retina. In case of discrepancy between changes in the central retinal thickness, vascular density and perfusion in at least one of the capillary plexuses of the retina and the photosensitivity of the macular region to the above parameters laser coagulation of the retina and/or intravitreal injections of anti-VEGF medications are performed to stabilize the course of diabetic retinopathy.
EFFECT: predicting the development of diabetic retinopathy in combination with diagnosis of the state of the retina of patients in the postoperative period of combined kidney and pancreas transplantation, ensuring the possibility of prescribing timely adequate therapy.
