SUBSTANCE: invention relates to medicine, namely to colorectal surgery. The rectum is resected at the level of the proximal sections of the internal sphincter and a J-shaped reservoir is formed from the retractable stump. A latex device 20 cm long is inserted into the reducible stump through the previously applied colotomy hole, which in its proximal part has the form of a cone with a base diameter of 35 mm and then narrows to 10 mm and continues in the form of a tube of the same diameter. A purse-string suture is applied to the intestinal wall around the colotomy opening at the level of the tapering part of the device that passes into the tube and tightened. Around the circumference with the capture of the serous, muscular and submucosal layers at the level of the base of the device 5 mm above the purse-string suture, 6 ligatures are applied from the inside to the outside. The same ligatures from the outside to the inside with the capture of the muscles of the internal sphincter and the mucous membrane of the anal canal are stitched into the anastomosed area. Biological adhesive is circularly applied to the serous surface of the proximal stump at the level of the base of the device. The superimposed ligatures are brought out through the anal canal, and the device is pulled down until the anastomosed areas come into contact. Ligatures are tied, the device is pressed against the anal canal and fixed to the skin of the perineum.

EFFECT: method allows to reduce traumatism, postoperative complications, accelerate rehabilitation in the postoperative period, and reduce treatment time.
