FIELD: medicine; paediatric surgery.

SUBSTANCE: diagnostic laparoscopy is performed with a visual assessment of the condition of the colon and small intestine. In the absence of endoscopic signs of active inflammation in the prestenotic part of the colon, a section of the intestine with stenosis is resected. Then, a histological examination of the edges of the resected fragment of the intestine is carried out using the cryotomy method. A colonic anastomosis is formed with the removal of the ileostomy. After healing of the anastomosis, the second stage of surgical treatment is carried out to restore the continuity of the gastrointestinal tract. When the endoscopy helps to visualize active inflammation in the prestenatic part of the colon, an ileostomy is formed. After stopping the inflammation, the second stage of surgical treatment is carried out — resection of a limited section of the intestine with stenosis is performed with intraoperative histological examination of the edges of the resected fragment of the intestine by cryotomy and the formation of an anastomosis. After healing of the anastomosis, the third stage of surgical treatment is carried out to restore the continuity of the gastrointestinal tract.

EFFECT: method makes it possible to exclude the presence of active inflammation in the prestenatic colon and perform radical surgical intervention, form an anastomosis within healthy tissues, avoid extensive resection that significantly reduces the risk of such postoperative complications as anastomosis failure and the development of peritonitis.
