FIELD: medicine; coloproctology.
SUBSTANCE: laparoscopic resection of the ileocecal angle is performed with extended resection of the mesentery with the intersection of the ileocolic artery distal 3-5 mm from the mouth of the artery and the intersection of the vein 4-6 mm proximal to the origin of the superior mesenteric vein, respectively. The peritoneum of the right lateral canal is dissected. Resection of the mesentery of the ileum and ascending colon is performed, leaving the lymphovascular pedicle in the preparation, followed by the formation of an isoperistaltic ileoascendoanastomosis.
EFFECT: reducing the risks of endoscopic and clinical relapse in the anastomotic area, as well as increasing the time until the development of disease relapse or excluding it during 3-year follow-up, simplifying surgical intervention, reducing the duration of the operation, and also reducing the risk of intraoperative vascular damage due to the intersection of blood vessels grounds.
