FIELD: orthodontic therapy.
SUBSTANCE: apparatus for conservative reposition of displaced condylar processes in case of incomplete fracture of the lower jaw in children during the period of mixed dentition contains fixators, a flexible elastic removable rod for installation on fixators, an additional flexible elastic removable rod for installation on fixators, orthodontic rings for the molars of the upper jaw connected by a wire metal palatine byugel. Orthodontic fastening ring for the maxillary molar is connected to the palatal guide. Orthodontic rings for the molars of the lower jaw are connected by a metal lingual arch. Orthodontic fastening ring on the mandibular molar is connected to the lingual guide. The fixators are made in the form of metal hooks 3.5–4 mm long with plastic end thickenings 2–2.5 mm in diameter. On the outer surface of the palatine guide and on the inner surface of the opposite side of the arch of the palatine clasp at the orthodontic ring, the said hooks are fixed with an orientation to the lingual arch. On the outer surface of the lingual guide and on the inner surface of the opposite side of the lingual arch near the orthodontic ring, the said hooks are fixed with an orientation to the palatine clasp. The metal hooks fixed on the outer surface of the lingual guide can be located on the side of the jaw displacement, opposite to the metal hooks fixed on the outer surface of the palatal guide of the palatine clasp. A method for conservative treatment of children in the period of mixed dentition with a displaced condylar process with an incomplete fracture of the lower jaw with the above apparatus includes fixing the apparatus with glass ionomer cement on the abutment teeth, after preliminary mechanical and medical treatment, by means of orthodontic fixing rings. When fixing the apparatus, metal hooks fixed on the outer surface of the lingual guide are placed on the side of the jaw displacement opposite to the metal hooks fixed on the outer surface of the vestibular guide of the palatine clasp. The installation of elastic removable rods on hooks is carried out the next day for constant wear for at least one month.
EFFECT: inventions make it possible to carry out conservative reposition of displaced condylar processes in case of incomplete fracture of the lower jaw due to the physiological traction of the chewing muscles, unloading the temporomandibular joint, relieving spasm of the chewing muscles and eliminating the displacement of the lower jaw towards the fracture, both when opening the mouth and in the state of physiological occlusion.
