FIELD: medicine; cardiac surgery.
SUBSTANCE: reconstruction of the proximal aortic arch is carried out with autologous aortic tissues. An oblique transection of the proximal segment of the aortic arch between the brachiocephalic trunk and the left common carotid artery is performed in the direction to the orifice of the left common carotid artery with the incision being extended distally at the orifice of the left common carotid artery along the lateral surface facing the brachiocephalic trunk, and then the incision is extended proximally to the ascending aorta along the inner radius. Counter displacement of the autotissue flaps of the transected proximal segment of the arch is performed and the outer flap of the transected proximal segment of the aortic arch is united with the dissected proximal section of the left common carotid artery by anastomosis, and the internal flap of the proximal segment of the aortic arch — with the distal segment of the ascending aorta dissected along the inner radius, with preservation of the intact inner semicircle of the aortic arch.
EFFECT: method allows to eliminate hypoplasia of the proximal segment of the aortic arch due to the use of tissues of the left common carotid artery and reduce the tension during the formation of the anastomosis, reduce blood flow turbulence by improving the anatomical shape of the aortic arch, as well as reduce the risk of injury to the left recurrent nerve and eliminate the risk of narrowing or deformity brachiocephalic trunk.
