FIELD: medicine; anesthesiology.
SUBSTANCE: invention can be used to prevent purulent complications in patients with periprosthetic infection of the hip joint at the stage of revision arthroplasty with an articulating spacer. Enteral nutrition is applied as follows. 3 days before the operation, the concentration of transthyretin is determined and the immune enteral nutrition Nutricomp Immune Liquid is used in a volume of at least 500 ml per day for 10 days. 3 days before surgery, with transthyretin values within the reference values, 500 ml per day of immune enteral nutrition is used, with transthyretin values below the reference values, the dose of enteral nutrition is determined based on the calorie deficit, but not less than 500 ml per day. The surgery is performed under combined spinal-epidural anesthesia with prolonged epidural anesthesia after the operation for 3 days. Immune enteral nutrition after surgery is started after the resolution of the spinal block in the absence of the following contraindications: hemodynamic instability, residual sedation, nausea, vomiting. The concentration of transthyretin is determined on the 1st, 3rd, 5th, 7th days after the surgery. If the concentration of transthyretin on the 1st, 3rd, 5th day after the surgery corresponds to the reference values, then 500 ml of the mixture is used per day. If the concentration of transthyretin on the 1st, 3rd, 5th day after the surgery is less than the lower limit of the reference values with the initial value of the transthyretin concentration greater than or equal to the lower limit of the reference values, or less than the initial value with the initial value of the transthyretin concentration less than the lower limit of the reference values values, then the dose of immune enteral nutrition is increased by 250 ml. If the concentration of transthyretin on the 7th day is less than the lower limit of the reference values, then immune enteral nutrition is continued for another 3 days at the last dose.
EFFECT: method enables the prevention of recurrent purulent complications and increases the effectiveness of treatment through the use of Nutricomp Immune Liquid immune enteral nutrition in an amount corresponding to a certain concentration of transthyretin.
