FIELD: medicine; ophthalmology.

SUBSTANCE: microfistula is formed in the trabeculo-descemet membrane (TDM) and an iris coloboma by the action of YAG laser radiation pulses with a wavelength of 1,064 nm, using laser applications. Previously, 1–2 mm from the zone of non-penetrating deep sclerectomy (NPDS) on the periphery of the iris, iridoplasty is performed, applying coagulates using a photocoagulator with a wavelength of 532 nm, a spot with the diameter of 100–200 microns, an exposure time of 0.2 s, a power of 300–500 mW, the number of coagulates 10–15. Further, in the projection of TDM, during laser descemetogoniopuncture, a microfistula with a power of 3.0–3.2 mJ is formed, the number of applications is 5–7. Then, in the transition zone of the iris into the ciliary body, laser iridectomy is performed with the formation of an iris coloboma using a YAG laser with a wavelength of 1,064 nm, a power of 3.0–3.2 mJ, the number of applications is 5–10.

EFFECT: method allows to achieve stable compensation of ophthalmotonus by activating the area of the previously performed NPDS immediately after the operation, and by means of sequential combined laser exposure — in the long-term observation period.
