SUBSTANCE: 4 cycles of chemotherapy according to the FLOT scheme are carried out and after 6 weeks, in the absence of signs of progression, a proximal subtotal resection of the stomach is performed with double-track reconstruction and D2 lymph node dissection. During the operation, after drainage of the subhepatic space, two trocars are installed in the paraumbilical region and the left lateral quadrant. Laparosco and a nozzle for intra-abdominal chemotherapy are inserted into the trocars. Intraoperative intra-abdominal aerosol chemotherapy under pressure PIPAC is carried out with the following drugs: cisplatin at a dose of 10.5 mg/m² diluted in 150 ml NaCl 0.9%, and doxorubicin at a dose of 2.1 mg/m² diluted in 50 ml NaCl 0,9%. Exposure is maintained for 30 minutes under normothermic conditions. 6 weeks after surgical treatment, 4 cycles of adjuvant chemotherapy are performed according to the FLOT scheme.

EFFECT: method allows to implement the principles of organ-preserving treatment, reduce the number of postoperative complications, as well as maintain the physiological and nutritional status of the patient, improve the quality of life of patients, reduce the risk of developing peritoneal carcinomatosis, reduce the frequency of reflux esophagitis and, as a result, increase survival rates.
