METHOD OF MEDIAGASTRIC RESECTION IN GASTRIC BODY CANCER T1-2 Russian patent published in 2025 - IPC A61B17/11 A61B17/68 

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to abdominal surgery, gastroenterology, oncology, and can be used in treating gastric body cancer. Gastric resection is performed along the greater and lesser curvature from the oesophageal-gastric junction to the pylorus in the block with greater and lesser omentum; the resection border is taken in the proximal direction – the spleen gate, in the distal direction – in the proximal direction from the pylorus. Resection is preceded by ligation of the left gastroepiploic artery, 2 short gastro-splenic arteries, right gastric artery, left gastric artery and vein. Two stumps are formed from the distal and proximal parts of the stomach in form of tubular structures of equal width, the distal stump is formed on vascular pedicles from the right gastroepiploic arteries, while the proximal stump is formed on short gastro-splenic arteries. Gastrophrenic ligament is transected, gastric fundus is released. Circular fundoplication cuff is formed by stitching, with one suture, the anterior and posterior walls of the stomach at the level of the cardia to the oesophagus from the lesser curvature, then anterior and posterior walls of the stomach are anchored to the oesophagus with seroserous sutures above the formed cuff. End-to-end gastrogastroanastomosis is formed.

EFFECT: method enables achieving oncological radicalism of the operation due to a greater gastric resection, eliminating the development of reflux oesophagitis, evacuation disorders of the upper gastrointestinal tract due to the formation of gastric stumps in the form of tubes and fundoplication cuff.
