FIELD: phthisiology.
SUBSTANCE: used to select shortened chemotherapy regimens in the treatment of pulmonary tuberculosis with preliminary comprehensive diagnosis of the tuberculosis process. Conduct laboratory diagnostics, X-ray examination, including thoracic region CT, sputum analysis to detect MBT 3 times, diagnostic bronchoscopy, identification of the drug resistance spectrum of the pathogen, assessment of the presence of concomitant diseases and their condition and assessment of the presence of possible contraindications to the prescription of drugs included in the shortened chemotherapy regimen. Assign a five-component starting combination for 9 months. In the absence of contraindications to the prescription of the anti-tuberculosis drug Delamanid and preserved drug sensitivity to fluoroquinolones, the Delamanid (Dim), Bedaquiline (Bq) regimen is used. Linezolid (Lzd), Cycloserine/Therisidone (Cs/Trd), Levofloxacin (Lfx). In the absence of contraindications to the anti-tuberculosis drug Delamanid and the presence of drug resistance to fluoroquinolones, the Dim, Bq, Lzd, Cs/Trd, Amikacin (Am)/Capreomycin (Cap) regimen is used. If there are contraindications to the use of Delamanid and the injection drug Am/Cap, the regimen Bq, Lzd, Levofloxacin (Lfx) / Moxifloxacin (Mfx), Cs/Trd, Pyrazinamide (Z) is used. If there are contraindications to the prescription of Delamanid and the possibility of introducing the injection drug Am or Sar into the regimen, the regimen Bq, Lzd, Lfx/Mfx, Cs/Trd, Am/Cap is used.
EFFECT: effective treatment while reducing the possibility of adverse reactions due to the choice of shortened chemotherapy regimens.
