FIELD: medicine.
SUBSTANCE: invention is related to traumatology and orthopaedics, and can be used in surgical treatment of posterior fracture-dislocations of the humeral head. Access is gained to the deltopectoral space and the head of the humerus is mobilized with preliminary suturing and cutting off the subscapularis muscle of the shoulder. Then open reduction of the humeral head is performed with dislocation of the humeral head into the surgical wound and the size of the humeral bone defect is determined. Next, access is gained to the ridge, the periosteum is dissected, and an autograft of the selected size is collected. The autograft is modelled and placed at the site of the defect with fixation using capless screws, and final processing of the autograft is performed to form the desired anatomical shape and curvature. Then the head of the humerus is reduced, the range of motion in the shoulder joint is checked, the subscapularis tendon is re-fixed and wounds are sutured layer-by-layer with the upper limb being fixed with a shoulder bandage in the position of shoulder adduction and internal rotation for a period of 6 weeks with limited movement in the shoulder joint.
EFFECT: method makes it possible to compensate for defect in the bone tissue of the head of the humerus, ensures the achievement of a large volume of active movements of the head of the humerus while simultaneously counteracting recurrent dislocations of the humerus, prevents the development of arthrosis of the shoulder joint in the early stages, reduces pain, reduces trauma during surgical treatment, and also early social rehabilitation of the patient while simultaneously increasing the quality of life.
