METHOD OF DETERMINING VIABILITY OF REPOSITIONED FLAP OF NASAL SEPTUM MUCOSA Russian patent published in 2023 - IPC A61B8/00 A61B17/24

Abstract RU 2807147 C1

FIELD: medicine.

SUBSTANCE: invention is related to ultrasound diagnostics and otorhinolaryngology. The perforation area of the nasal cavity septum is examined using an ultrasound scanner using linear sensors with a frequency of 6–15 MHz. The sensors are installed on the skin along and below the bridge of the nose in the cartilaginous part of the nasal septum, on the wing of the nose. After preliminary application of ultrasound gel to the surface of the skin and 1–2 ml into the nasal cavity, the wing of the nose is pressed with a sensor to the nasal septum. The area of perforation of the nasal septum (NS) is scanned in B-mode. The location of the perforation, its length and width in mm are determined. Duplex color scanning is turned on to identify the network of blood vessels above, around the perforation and below the perforation. The maximum blood flow velocity in the arterial vessels Vmax in cm/s and the resistance index Ri in the area of intended movement of the flap are determined. The coefficient of vascularization of CV is calculated as the ratio of Vmax to Ri:CV = Vmax:Ri in cm/s. Studies are carried out sequentially on the right and left sides of the nasal septum. The results of the study with images are included in the ultrasound examination protocol. CV values of 30–60 cm/s included in the positive range of values, ensure successful movement of the flap to close the PN defect.

EFFECT: method makes it possible to determine the viability of a repositioned flap of the nasal septum mucosa by creating new criteria for assessing the blood supply to the nasal septum in the perforation zone.
