FIELD: medicine; cardiovascular surgery.
SUBSTANCE: 3 cm long skin incision is made along one side of the xiphoid process. Then tissue retraction is carried out using a Cushing-Landolt nasal dilator, achieving visualization of the pericardium under direct visual control. Then pericardiotomy is performed using a high-frequency electric coagulator knife with a scalpel electrode more than 10 cm long. A drainage tube is installed in the pericardial cavity through a separate access, 1 cm away from the edge of the skin incision, and the drainage is secured using a U-shaped suture.
EFFECT: method allows to reduce the time of surgical intervention by using a Cushing-Landolt nasal dilator for tissue retraction and ensuring optimal visualization under direct visual control, as well as to reduce trauma to surrounding tissues by making a 3 cm long skin incision on one side of the xiphoid process.
