FIELD: neurosurgery.
SUBSTANCE: group of inventions relates to the methods for manufacturing implants for plastic surgery of skull bone defects and devices for their implementation. A method of manufacturing individual neurosurgical implants for plastic surgery of skull bone defects includes preoperative and intraoperative stages and is characterized by the following: at the preoperative stage, computer-implemented modeling of the implant is performed in accordance with the shape and size of the bone defect and modeling of the device for making molds based on the relief of the outer surface model of the implant, then the device is manufactured using 3D printing and self-hardening medical silicone is poured into the molding cavities of its parts. Upon completion of polymerization, the resulting parts of the mold for manufacturing implants are removed from this device and subjected to sterilization. At the intraoperative stage, under aseptic conditions, sterile acrylic cement is placed into the mold; upon completion of polymerization, the resulting implant is removed from the mold and used for plastic surgery of the bone defect. A device for making molds includes two parts that form a molding cavity following the relief of the outer surface of the implant for reconstruction of a bone defect, and the said device is characterized by containing two parts, each of which is made of a rigid material and contains side walls and a molding cavity, the bottom of which repeats the surface relief of the implant for reconstruction of a bone defect and includes a surface for joining parts of the mold. The side walls have a height of not less than 15 mm from the highest point of the bottom and a thickness of 1.5–2 mm and are made with an expansion towards the base at an angle of 55–75 degrees and a height of 15–20 mm. The mating surface of the mold parts has a width of at least 10 mm and contains projections and recesses for congruent engagement of the mold parts. The device for manufacturing implants is a mold that includes two detachable parts, upper and lower, forming a molding cavity that repeats the inverted relief of the implant surface for reconstruction of a bone defect, and is characterized by the following: each part is made of medical silicone and has a thickness of at least 15 mm at its thinnest part, wherein the mating surface of the mold parts is at least 10 mm wide, contains projections and recesses for congruent coupling of the mold parts, as well as cutouts for the drainage of excess cement.
EFFECT: use of the invention makes it possible to increase the efficiency of manufacturing implants for plastic surgery of skull bone defects.
