FIELD: medicine; neurosurgery.
SUBSTANCE: preoperative computer-implemented modeling of the volume of resection of a tumor affecting the bone structures of the cranio-orbital region is carried out. An individual stencil is made for intraoperative identification of the outer boundaries of the resection zone of superficial bone structures. An individual implant is made to reconstruct the bone defect. The implant is formed in such a way that its outer contours contain protrusions and/or depressions to match the corresponding irregularities of the edges of the bone defect, and the volume of the affected orbit after surgery becomes 3–5% larger than the volume of the healthy orbit. Intraoperatively, the stencil is positioned and the outer boundaries of the zone of resection of superficial bone structures are marked. Resection of superficial bone structures is performed. Resection of deep bone structures is performed with identification of the boundaries of the resection zone using frameless neuronavigation until the planned volume of tumor removal is achieved. The implant is positioned until optimal congruence between the contours of the implant and the edges of the bone defect is achieved. The implant is fixed using amagnetic fasteners in at least three areas: frontal, temporal and the area at the base of the frontal process of the zygomatic bone.
EFFECT: method makes it possible to increase the efficiency of operations to remove tumors of the cranio-orbital zone and reconstruct the resulting bone defect.
