FIELD: medicine; operative gynecology.
SUBSTANCE: if single or multiple myomatous nodes measuring at least 5 centimeters in size are detected, myomectomy is performed with temporary occlusion of the vessels supplying the uterus. In this case, the uterus is dissected by opening the peritoneum at the site where the uterine artery originates from the internal iliac artery in the projection of the ovarian fossae on both sides. Before the stage of enucleation of myomatous nodes, Doppler indicators of the uterine arteries and vessels of the perinodular region are measured using a flexible laparoscopic sensor. Atraumatic 8 mm titanium clips are applied to the uterine arteries and, based on the infundibulo-pelvic ligaments, repeated Doppler measurements are performed, and the myomatous node or nodes are enucleated. After removing the temporary clips from the uterine arteries and infundibulo-pelvic ligaments, the laparoscopic sensor is reinstalled and Doppler measurements are performed.
EFFECT: increasing the accuracy of diagnosis and reducing the amount of intraoperative blood loss, increasing the effectiveness of surgical treatment.
