FIELD: medicine; otorhinolaryngology.
SUBSTANCE: at the stage of stapedotomy, the niche of the oval window is widened and the base of the stapes is thinned using at least one microbur with a cutter diameter of 0.5 to 1 mm and a rotation speed of 6,000 rpm. Fenestration for the purpose of installing a stapes prosthesis is performed using a pulsed CO2 laser with a defocused beam with a diameter of 0.6–1.1 mm, a power of 26–27 W, a pulse duration of 0.05–0.08 s. At least 1 pulse is produced.
EFFECT: method allows to reduce the risk of intraoperative and postoperative complications in patients with an obliterating form of otosclerosis with II–III degree of obliteration of the oval window niche, reduce the bone-air interval, maintain bone conduction thresholds at the preoperative level, significantly reduce the risk of labyrinthine complications and recurrence of hearing loss.
