FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to otolaryngology, and can be used in the surgical treatment of patients with a cochlear form of otosclerosis. Produce tympanotomy. Remove the bone shelf over the stirrup. Conduct an operative exploration of the tympanic cavity, and then stapedotomy. Fenestra vestibule fossa is formed, which is corresponding in size to the norm, by removing otosclerosis foci. Then remove at least 2/3 of the footplate of the stirrup. Next, remove 40–50 mm3 of perilymph, replacing it with the same volume of sterile saline solution, which is injected into the vestibulum with a syringe for injection. Then, from the back of the foot, an auto-tissue of the vein is taken through the incision and a graft of 3×5 mm is formed. Wound on the foot is sutured with interrupted sutures. From the region of the posterior surface of the auricle, an autocartilage tissue is taken through an additional incision, from which an autocartilage prosthesis of stirrup 3.5 mm long is formed. Wound on the posterior surface of the auricle is sutured with interrupted sutures. In vestibule fossa, a prepared vein is laid out from the back of the foot, with intima is positioned towards the vestibulum, and a formed autocartilage prosthesis of the stirrup is placed under the sesamoid bone of the long crus of incus. Next, 1 ml of sterile warm hydrocortisone solution for injection is injected into the tympanic cavity. Meatotympanic flap is placed back. On the skin incision of the auditory meatus, a rubber strip is placed. Swab the auditory meatus with cotton wool tampon or a gelatin sponge with an antibiotic.
EFFECT: method allows to obtain a positive functional result in patients with a cochlear form of otosclerosis by eliminating the cause of the conductive component of hearing loss and a positive effect on the sensorineural component, namely, improving the bone sound conduction, as well as improving the methods of stapedoplasty with autocartilage on the vein by partially replacing the perilymph with a physiological solution.
