FIELD: medicine; otorhinolaryngology.
SUBSTANCE: under visual control, using a cold plasma coblator electrode moved along the lacuna adjacent to the lobule to be removed on its lateral side, the lobules are cut off one by one from the basal part of the pharyngeal tonsil by dissection at the base of the lobules at the level of the bottom of the lacunae. The lobules are removed in the following order: central, paracentral, lateral lobules, directing the working surface of the electrode medially to the lobule being removed.
EFFECT: method eliminates the risk of unintentional penetration of a surgical instrument into the retropharyngeal space and excessive radicalism of the operation, preserves part of the functional tissue of the pharyngeal tonsil, avoiding the risk of relapse of adenoiditis, eliminates the risk of proximal occlusion of lacunae of the pharyngeal tonsil and preserves a clinically significant focus of infection and relapse of tonsil hypertrophy, reduces or eliminates putrefactive odor from the wound surface in the postoperative period, eliminates muscle pain in the neck after surgery, and also allows to perform histological examination of the tissue of the removed lobules.
