FIELD: medicine; surgery.

SUBSTANCE: invention relates to the choice of tactics for managing laparotomy wounds of the anterior abdominal wall for gunshot wounds of the abdomen. The method includes the use of a scale for a comprehensive assessment of the reparative potential and the risk of developing infectious complications, including the determination of the following indicators: the presence of concomitant diseases in the patient: HIV, viral hepatitis B and C, diabetes mellitus, chronic kidney disease, obesity; hemoglobin less than 100 g/l, total protein less than 50 g/l, leukocytosis more than 15×109/l, C-reactive protein more than 100 mg/l; the presence of fibrin and/or blood clots in the laparotomy wound; the presence of relaparotomy and/or intestinal stoma. Then the results obtained are scored: the patient’s presence of the above indicators is assessed as one point. Next, the scores are summed up, after which the results are interpreted. If the score is 4 or more, the low reparative potential of tissues and the high risk of infectious complications are assessed, and vacuum therapy of the laparotomy wound is performed. With a score of 2 to 3, the healing of the laparotomy wound is assessed as average and the skin is sutured with rare sutures with mandatory drainage of the subcutaneous fat. If the score is 1 or less, good healing is assessed and layer-by-layer suturing of the laparotomy wound is performed.

EFFECT: invention makes it possible to objectify the assessment of the risk of developing infectious complications from the laparotomy wound and timely adjust the tactics of its further management.
