FIELD: medicine; neurosurgery.
SUBSTANCE: invention can be used to prevent postoperative swelling of the soft tissues of the orbit. Surgical access is performed to the tumor-affected structures of the orbit and base of the skull. The tumor is removed. Reconstruction of the bone defect is performed. The wound is closed. A canal is formed in the lateral wall of the orbit. The distal end of a thin elastic catheter made of a bioinert polymer is perforated and inserted through the indicated channel into the orbit, placing it in the fatty tissue between the superior and lateral rectus muscles of the eye. A counter-aperture is applied in the posterior frontal or parietal region and a straight tunnel is made under the temporal muscle in the direction from the orbit to the counter-aperture. The proximal end of the catheter is passed through the tunnel towards the contraperture and brought out. The proximal end of the catheter is treated with an antiseptic and a sealing plug is placed on it. In the postoperative period, a solution of glucocorticoids is injected into the catheter, with the first injection performed immediately after surgery, and subsequent injections are performed once a day for five days. Upon completion of the course of glucocorticoid administration, the catheter is removed.
EFFECT: invention allows to minimize the side effects of glucocorticoids and provides effective prevention of postoperative swelling of the soft tissues of the orbit.
