FIELD: medicine; anaesthesiology.

SUBSTANCE: at the first stage, the patient undergoes a retrobulbar block using an anaesthetic mixture in a dose of 2.0-3.0 ml containing 1.0-1.5 ml of a lidocaine solution with a concentration of 20 mg/ml and 1.0-1.5 ml of a bupivacaine solution with a concentration of 5 mg/ml. At the second stage, a pterygopalatine blockade is performed by introducing an anaesthetic mixture containing 1.0 ml of a dexamethasone solution with a concentration of 4 mg/ml and 6.0 ml of a ropivacaine solution with a concentration of 7.5 mg/ml. In this case, the pterygopalatine blockade is carried out by inserting a needle into the area of the angle formed by the zygomatic arch and the coronoid process of the mandible along the anterior edge of the masticatory muscle. Next, the needle is moved inward and upward at an angle of 60° to the surface of the skin of the cheek with the needle tip directed to the outer edge of the orbit and 4.0-5.0 ml of anaesthetic mixture is injected into the pterygopalatine fossa. The needle angle can be changed up to 30° and directed to the pupil of the eyeball until a sensation of “falling” is obtained and 2.0-3.0 ml of anaesthetic mixture is injected into the lower orbital fissure. Irradiation of intraocular tumours is carried out using a Gamma Knife installation.

EFFECT: method allows you to increase the time (up to 6 hours) of the analgesic effect throughout the entire process of irradiation of intraocular tumours, as well as eliminate pain after the procedure.
