FIELD: medicine; radiation diagnostics; traumatology and orthopedics.
SUBSTANCE: invention can be used to select surgical treatment for chronic instability of the shoulder joint. Using functional multislice computed tomography, the joint is examined with the patient lying on his back, with the upper limb abducted and the elbow joint flexed at an angle of 80 to 90 degrees, with further rotation of the upper limb for 6–8 seconds, with a slice thickness of 1 mm, when performing sagittal reconstruction. At the rotation stage, the point of maximum contact of the humeral head defect with the glenoid articular track is determined and marked. Using the established mark, the localization of the defect of the head of the humerus relative to the marginal part of the glenoid cavity of the scapula is assessed. When the defect of the humeral head is localized medial to the marginal part of the glenoid cavity, an osteoplastic operation is chosen, according to the Bristow-Latarget type, and when the defect of the humeral head is localized lateral to the marginal part of the glenoid cavity, a soft tissue operation is chosen, according to the Bankart type.
EFFECT: method provides the ability to predict the degree of risk of developing recurrent dislocations in the future, and also allows to plan the required surgical treatment by accurately assessing the relationship of the humeral head defect with the glenoid articular track at each stage of rotation of the upper limb from an abduction position simulating a conditional dislocation.
